Hannah Marie JerzakWas born September 16, 1999 in Elmira, the youngest daughter of Amy L. (Brooks) and Tony M. Jerzak, passed away very unexpectedly on Sunday, August 9, 2015 at the age of 15. She is survived by her loving parents: Amy and Tony; sisters: Tricia Ashley and her children: Dana, Tristan and Jackie; Brittany Jerzak (fiancée Justin White) and Emily Jerzak (fiancée Andrew Van Skiver) and their son: Andrew; paternal grandparents: Gary and Susan Jerzak; maternal grandmother: Sherry (Joey) Vincent; maternal grandfathers: Edwin Brooks and PaPa Abraham Crawley; along with several other relatives and many friends. Hannah was always willing to lend a hand and help others. She volunteered frequently and was especially fond of volunteering at The Bridge in Sayre, PA. Hannah was an outstanding softball player with Southport Recreation Association and was looking forward to her Junior year at Elmira High. She absolutely loved country music and her camouflage. Family and friends are invited to gather at the Olthof Funeral Home, Inc. 1050 Pennsylvania Ave. Elmira/Southport, NY on Thursday, August 13, 2015 from 12:00 to 3:00 PM and 5:00 to 7:00 PM. Funeral Services celebrating Hannah's life will follow the visitation at 7 PM. Reverend Chip Possee will officiate. Interment will take place on Friday, August 14, 2015 in Forest Lawn Memorial Park at 10:00 AM.
