John S. Forrest, M.D. died, at home surrounded by his loving family, August 28, 2015. Born in Nyack, NY on July 6, 1929 to Jean and Albert Forrest, Jack is survived by his loving wife of 64 years, Judith Marshall Forrest. He is also survived by a sister, Nancy Forrest Lange and a foster sister, Sally (Greg) Mann and foster brother Jack Anderson. He was blessed to be the father to 5 children whom he was proud of and so enjoyed, David (Lucy) Forrest of Elmira, NY; Paul (Mary Eileen) Forrest of Delmar, NY, Steven (Linda) Forrest of Elmira, NY, Deborah (Albert) Ash . of Utica, NY and Rebecca (William) Schlag of Chardon, Ohio. He was also blessed to have 15 grandchildren, Sarah Forrest, Peter (Katrina) Forrest, Michael Forrest, Matthew Forrest, Alaina (Robert) Laferriere, Daniel Forrest, Andrew (Allison) Forrest, John Forrest, Rachel (Joseph) Cloke, Timothy Forrest, Nathan Forrest, Katharine (John) Herzig, Robert Ash, Jacqueline Schlag and Margaret Schlag, as well as 2 great-grandchildren,Vivian Laferriere and Calvin Laferriere . Jack Forrest graduated from Syracuse University in 1950 and Upstate Medical School of Syracuse in 1953. His internship and 1st year surgical residency were at Syracuse from 1953-1955. In 1955 he entered the U.S. Air Force as a reservist and then served as a regular Air Force officer from 1957-1966. During his time with the service, his family lived in many places including Missouri, California, North Carolina, South Carolina, and England. In 1966 he resigned from the service as a Major and entered private practice with Elmira Orthopedics. He retired from surgery in 1992 but, continued working doing orthopedic evaluations for Riverfront Medical until 2008. He was past president of the American Cancer Society, Elmira Rotary, Arnot Medical Staff and the Chemung County Medical Society. He was also active on behalf of the Elmira Symphony & Choral, Tanglewood Nature Center and multiple medical societies. A lifelong Methodist, he was an active member of Westside United Methodist Church since moving to Elmira in 1966.Jack ran track at Syracuse and remained active in sports, playing basketball, volleyball, softball and most passionately tennis with the Elmira Tennis Association, which he played well into his 70's. Jack was blessed to have many friends as well as many loyal patients from his medical practice who, along with his wife and family, greatly enriched his life. Family and friends are invited to gather at the Olthof Funeral Home, Inc. 1050 Pennsylvania Ave. Elmira/Southport, NY on Tuesday, September 1, 2015 from 4:00 to 7:00 PM. A Memorial Service celebrating Jack's life will take place in the Westside United Methodist Church 744 W. First St. Elmira, NY 14905 on Wednesday, September 2nd at 11:00 AM. The Rev. Dr. Vincent Howell will officiate. Interment will follow in Woodlawn Cemetery with Rev. Howell conducting the committal prayers. The Chemung County Veteran's Honor Squad will accord Mr. Forrest with Military Honors at the grave. In lieu of flowers, donations in his name may be made to Westside United Methodist Church, Tanglewood Community Nature Center, Inc. 443 Coleman Ave. Elmira, NY 14905 or the charity of your choice.
